Note: All paperwork for funding is due on or prior to December 31, 2025. Paperwork received after the due date may result in partial or no funding. Scholars are only eligible to receive funding for the current school year.

Important: Before you begin the upload process, please gather ALL required documents listed below. You will upload all documents together in a single form submission.

File Requirements:

  • All documents must be uploaded as individual PDF files

  • Please follow the specified naming convention for each document

  • The year should be the year you plan to graduate from college

  1. Senior Year Info Form

    • Complete the form and save as a PDF

  2. Certification and Verification Form​

    • Click here to access and download the Certification and Verification fillable PDF 

    • Complete the form and save as a PDF

  3. Current Unofficial Transcript

    • Current unofficial transcript that includes your Fall 2025 grades

    • Your name must be visible on the document

    • File Naming Convention: LastName_FirstName_Year_UnofficialTranscript.pdf

    • (e.g., Smith_John_2026_UnofficialTranscript.pdf)

  4. Handwritten Thank You Note

    • Write a handwritten thank you note to the Trustees of Anonymous Trust, scan it, and save as a PDF

    • File Naming Convention: LastName_FirstName_Year_ThankYouNote.pdf

    • (e.g., Smith_John_2026_ThankYouNote.pdf)

  5. Level All Module

    • Click here to access the Level All module

    • Complete the Level All module and take a screenshot of your scores

    • Save the screenshot as a PDF to upload

    • File Naming Convention: LastName_FirstName_Year_LevelAllScores.pdf

    • (e.g., Smith_John_2026_LevelAllScores.pdf)

  6. Level All Reflection Questions

    • After you complete the Level All Modules, write a 3-4 sentence reflection on each module showing: 

    1. What are your thoughts and opinions on the modules?

    2. How will these knowledge and tools help you as a senior in college and as an adult moving forward after college?

    3. What actions are you willing to take to become the best version of yourself? What actions are you going to take now?

    • Save the document as a PDF to upload

  7. Spring 2026 Billing Statement

    • Your spring semester 2026 billing statement
